It was a combination of answer two and three.   I was removing a flake on the left side of the back and it overshot to the right side.  When I started this point the crack looked shallow. I did not think it went this deep. This flake went extra deep and because of the crack knocked off the other side.  I was very shocked and thought you all might enjoy seeing it.
